Resource Summary
This theory of change model for cross-sector alignment was developed by the Robert Wood Johnson Foundation (RWJF) and the Georgia Health Policy Center. It provides a framework for public health, health care and social service organizations collaborating to improve health, including factors that may lead to sustainability and long-lasting change from these partnerships.
