Lincoln County Health Department Customer Satisfaction Comment Card

This is an example of a customer satisfaction survey that could be used during the Implement and Improve Phase to evaluate how well staff are interacting with customers. It was developed by the Northwoods Shared Services Project in Wisconsin, a member of the Center’s Shared Services Learning Community.
