This conversation tool is for local health departments and health centers that are considering sharing services. It contains a series of questions organized under headings which represent areas that should be considered when assessing trust.
It is one resource from a suite of tools that supports the sharing of services – both clinical and nonclinical – between local health departments and health centers. The tools were developed collaboratively by the Center for Sharing Public Health Services and the National Association of County and City Health Officials (NACCHO), with support from the Health Resources and Services Administration (HRSA).
